hi guys my name is destiny prater i'm your guys school consultant um this is gonna be a short video on where to find your credit card authorization form and kind of what to do with it okay so this is a back office um you'll need to go to your back office um to get to your travel cafe if you look on the left hand side a couple tabs down right here it's your travel cafe there's so much information in your guys's travel cafe i really recommend going in there and just kind of clicking around there's trainings um all kinds of stuff forms um that you guys will need just all kinds of things that you guys can really just click around and watch some stuff that's really how you're going to get familiar with it as well as your back office do some clicking around okay so once your travel cafe loads um over here on the left hand side there's going to be a forums tab you need to click on forms okay once this is finished loading basically what you're going to do is click download and it's going to download in the bottom left hand corner you'll just click on it and it will pop up and this is what it's going to look like so this is the credit card authorization form um basically what you're going to do is edit it to be yours so up here independent contractor business name put your business name uh put the address your address um the city state and zip and then your telephone number and i also have my email as well okay so right here you would put your name where it says independent contractor that's where you put your name so basically um first let me tell you guys what this is for um the credit card authorization form is basically to protect you um if anything wasn't happen and somebody tried to say that you took money off their card without their permission this is how you're gonna uh protect yourself um that's why we have them sign this so basically what i normally do is fill it out for them besides the last four digits of the credit card obviously they have to fill that part out um i try to fill out most of the information for them um the questions that obviously you can't uh answer for them then you could uh just let them fill those parts out i try to put in the total charge to their card so they know exactly what's going to be charged and then they're going to sign it and date it down here okay so what they have to do is um they need to sign it and date it and send it back to you and they also need to send a picture of their id um so there's a couple ways that they can actually do...
