one medical plan additional features of the medical plan include surgery plus rally hinge health my health novel and aetna's able to program icuba's whole person approach triggers specialized outreach to members with specific health challenges by trained health advisors registered nurses and pharmacists offering support education and assistance to help you manage your condition a cuba offers very rich comprehensive benefits to their employees and their families many services are covered in full examples of these services can be seen in the first column such as office visits to a total care primary care physician colonoscopies both diagnostic and routine preventive services such as mammograms psa testing and immunizations including the new covid19 immunization are all covered in full other services require that you pay a copay or coinsurance example of these are found in the middle column such as advanced imaging chiropractic services and urgent care visits major services require that a deductible be paid examples of these services are listed in the column on the far right in patient hospitalizations surgeries and x-rays are all examples of services that are subject to the deductible blue cross blue shield's my health toolkit app can be downloaded from google play or the app store once downloaded you can choose to log in using a username and password or opt using the touch id feature instead when you are logged in you will be able to download and save a copy of your id card so that you will have access to it when you are offline the my health toolkit mobile app allows you to view the status of your recent claims or review your benefits there is also a feature that will allow you to search for participating providers so that you can stay in network and make the most of your benefit plan total care providers improve screenings medication management and other programs help provide better outcomes and lower costs for patients icuba members enjoy free office visits when seen by a total care family practice internal medicine or pediatric provider total care providers must meet rigorous quality standards to be part of this special designation of physicians in the blue cross blue shield network you can find total care providers when you log in to my health toolkit now let's switch gears and talk about the details of your plans the preferred plan is a co-insurance based plan the plan provides both in and out of network benefits for the purpose of this presentation i will be focusing on the left-hand column which reflects the in-network benefits it is important that you stay in-network whenever possible to maximize your benefit the preferred plan offers an individual deductible of twenty five hundred dollars or a family deductible of five thousand dollars the cost share or coinsurance under this plan is twenty percent the out of pocket maximum is four thousand dollars for an individual and eight thousand dollars for a family this is the maximum amount that you would pay for covered services during the plan year please note that the pro provider office visits are covered with a 20 coinsurance however if you choose to see a total care primary care physician previously known as a bdtc physician or blue distinction total care physician the office visit will be covered in full lab tests are also covered at 100 when provided by quest diagnostic labs hi jc from optimrx did you know that prescription drug coverage is included with your selection of an icuba medical plan prefer generic copays at the nsu pharmacies are zero dollars certain diabetic supplies are also free to use such as meters and lancets your coverage has a max out of pocket so you will never pay more than two thousand dollars for individual coverage or four thousand dollars for your family plan also please remember that you can save money by filling 90 day supplies of your maintenance medications if you have any questions don't hesitate to contact our customer services teams did you know that generic drugs on our preventative drug list are now at a zero copay to you these medications can help protect against or manage certain conditions by through aetna behavior health let's take a look at the variety of services available in member responsibility if you look under the first category you'll see employee assistance program also known as eap fitness employee assistance program provides up to six short-term counseling sessions per episode per year for everyday issues such as stress relationship issues grief and more this benefit is a free benefit at no cost to you at miss behavior health in network outpatient services members are responsible for 20 coinsurance with the exception of the able2 program which is at no cost to you let's take a look at the variety services under the outpatient category first you should see professional counseling this allows you to speak with a licensed clinician for any type of issues such as anxiety depression mood disorder trauma alcohol or substance abuse and more psychiatric consultation and medication management is typically when you meet with a psychiatrist that can prescribe and monitor medication to help you with your behavior health diagnosis applied behavioral analysis therapy related to autism spectrum disorder diagnosis does require authorization you may contact edna behavior health and speak with a clinician that specializes in autism who can assist families with addressing questions about your benefits the authorization process finding providers ensuring that the treatment is effective and connecting to available resources partial hospitalization is also known as php these programs include physician and nursing services as well as group individual family multi-family group psychotherapy and other services these programs are often used in lieu of an inpatient stay or as a transition from an inpatient stay typically step down program once you're discharged outpatient detoxification monitors withdrawal from alcohol or other substance of abuse they can administer medications that assist with detoxification and recovery from addiction intensive after your deductible is satisfied note that the eap plan and the able to benefits are not available as an out of network option all deductibles and out of pocket maximum amounts are combined with prescription drug plan document thank you next let's review your spending account options hello my name is charlie with discovery benefits a wex company and we administer your healthcare flexible spending account and your healthcare reimbursement accounts now these are great benefits that your employer offers to give you more control over your healthcare spending and it's our hope to make it really easy to use these great benefits so you can pay for your out-of-pocket expenses and move on with your lives so your employee offers two spending accounts which i'll go over in detail but before i jump in i want to note that the healthcare flexible spending account does require an active election and so what that means is every year you need to let your employer know that you like to participate and how much you'd like to set aside from your paycheck to fund this account but one thing that makes this account really great is that anything you take out of your paycheck lowers your taxable income it increases your take-home pay so your paychecks are a bit bigger and then you have this pool of money set aside to pay for out-of-pocket medical expenses so it's a really great plan that we hope you take advantage of but let's go into more detail about these two accounts and what similarities they have as well as a few very important differences we'll start with a healthcare flexible spending account this is like a bank account that you can use to pay for your out-of-pocket medical expenses both for you and your eligible dependents you can set aside up to 2 750 for these expenses and whatever amount you elect will be deducted from your paychecks evenly over the course of the plan year and what we do is give you a debit card that's loaded with your funds so you can pay for your expenses really easily most of the time you'll swipe your card and you're good to go but there will be times when we may ask for further documentation to make sure that what you have purchased meets irs requirements for eligible expenses but don't worry we will let you know what we need why we need it and how to get it to us what's really great about this spending account is that whatever you elect you can use right away and so if you elect the full two thousand seven hundred fifty dollars you can use that full two thousand seven hundred fifty dollars right away so if you have a large medical expense early in the plan year swipe your card file a claim up to the amount of your election even if you haven't made all of your payroll deductions yet you can still use the full amount but it's important to note that if the year goes by and you do not use your funds if you have anything left over then you do lose those funds so as you determine if you want to participate and how much you want to elect make a plan think ahead determine how much you think you'll need because once you make that election you cannot change it unless you experience a qualifying event now let's talk a little bit about your health care reimbursement accounts they're similar but there are also a few key differences i'd like to point out first the healthcare reimbursement accounts are tied to your medical plan and so if you're in the medical plan with icuba then you also have the healthcare reimbursement account if you're not in the medical plan then you do not have access to these funds another key difference is that rather than funding the account with money you take out of your paycheck this account is funded by your employer and they give you money every month that you can then use to pay for your health care expenses as well as the health care expenses for those enrolled in the iqbal medical plan with you and so this is a great account as well because it is employer money that you can use to offset your out-of-pocket costs and we do give you a debit card as well which makes it really easy to access those funds another important thing to note is that if you are in the medical plan for 36 months then the funds that your employer gives into your healthcare reimbursement account they start to roll over year over year for you to keep using so if you do not use funds in a given year then they're available to you in the next year and so that's an amazing incentive for you to take part in the cuba medical plan and so we hope you do and as again as you do you'll find that you have more control over your health care spending you're lowering your taxable income you're increasing your take-home pay and then you have these pools of money that you can use to pay for your health care expenses if you're in both accounts we will pull from your health care flexible paying account first before we pull from your health care reimbursement account and you'll have one screenings and the annual wellness exam and i really like to pair these into one bucket totaling one hundred dollars i go to my annual wellness exam where my doctor does my annual blood work and at that same point in time i can print out the physician results form and have my doctor complete the form while he's reviewing my blood work with me super quick super easy and two birds with one stone also we have added a few corn coin features previously we offered teledoc registration and tobacco at a station so those are unchanged but new this year you can earn coins for getting your flu shot an eye exam or a dental exam a brand new feature this year which will review more in the coming slides is my health novel so this is a way to get you engaged within um various programs that fit your needs and we'll talk about that a little bit more coming up but each employee and covered spouse has the ability to earn up to 330 this year so we want you to keep moving with the rally virtual city challenges and this is what i was talking about the incentive being added for five dollars per quarter that amazon gift card is still going to continue for 14 or 4 every 14 days a 50 amazon gift card is drawn at random from rally so if you're in there you're syncing your device and you're joining those challenges meeting those milestones you're entered every 14 days for that amazon gift card coming soon we're really excited and you guys have asked a lot for private city challenges so new this year icuba will be hosting private city challenges one per quarter and these will be icuba wide information coming about these private city challenges in the weeks and months to come so just give you a little sneak peek on how you can join these city challenges under the activities in the app you can click on activities and then the exercise button and then from there you will select group challenges and then you can choose which city challenge you'd like to join this shows the dc dash you would just click join they're done by a certain number of days and you'll it'll show the amount of completion that you still have the opportunity to get those milestones in so we also want you to know your numbers with your physician results form so this is tied back to the biometric form so step one employees and spouses enrolled in the icuba medical plan can visit your individual portal at the my quest for health and you can download the physician results form if you've never registered before you can use the registration key icuba er and the unique id is your blue cross blue shield id number the next step with the physician results form each physician results form contains a barcode that's specific to the employee or spouse so this cannot be shared it is unique to you so each employee and spouse need to set up their own account with my quest for health dot com and download that form a pro tip if you have an established relationship with your physician you can request your blood work ahead of time that's what i do i call my doctor's office when my annual visit is scheduled i ask for them to call in the blood work in advance i go to quest and i get my blood work completed and then all the same visit my doctor is able to review the blood work with me and complete that form all at the same time some items to keep in mind the screening will include cholesterol hdl ldl triglycerides blood sugar blood pressure in a1c so you need to also make sure that you're fasting when you get your blood work done remember to take the form with you to your annual physical it's important both you and your physician sign the form and then your doctor's office can fax it in with the number that's listed right on the form you're encouraged also to request a copy for your own records and if necessary you can also upload it to the quest portal in your own quest portal members should allow at least 30 days for the form to be uploaded in fact to see the results and the final deadline for submission to quest is at the end of the year no later than march 31st we also want you to stay informed with clinical rewards if you've been diagnosed with one of these chronic conditions shown here you're eligible for clinical rewards in rally what this means is you are provided quarterly and annual goals and just educational material related to your chronic condition so you will have a tracker feature within the rally system to help you keep track of how many points you've earned and various goals throughout the year your activities and ultimately the goal is for you to stay engaged with your chronic condition meet the quarterly and annual items that they're putting out for you and then 500 rally coins you are eligible to be earned each quarter and 2 000 rally coins per year the next newest and most exciting thing that we're rolling out this year is my health novel so with my health novel you must begin by taking a one minute assessment who doesn't have one minute right and from that you will receive recommendations for programs and resources for which you qualify just for completing the assessment and this engagement this will get earn you that rally incentive when you qualify and sign up you'll gain access to health coaching nutrition guidance digital tools group support and and a lot more to help keep you on track whether you're interested in
