do you smoke cigarettes no and then your pranksy test was negative today um what are you currently using for contraception the depo okay and then when was the last date that you got a shot here's a calendar if that would be helpful in February about the 17th okay and then how many years or months have you been using the depo for two years in two months okay and then what method are you interested in switching to um I've heard a lot about um I you D I would like to know more the hormonal IUD or the non hormonal I you do you're both I don't know they do okay now that's fine so you're in the right place we're going to give you information about all the methods available and then you can ask questions and then you can choose which method sounds the like of the best option for you okay sounds good so looking at again at this calendar um when was the first day of your last menstrual period and we're looking at today is here we are in May you remember the first day that you started um honestly I haven't had a cycle since three months after I started the depo okay and so let's see go back to maybe like passing 11 I would say about May of 2011 and then think back to what your parody like before you were on the depo so back to like what your natural periods are like okay um were they regular or irregular they were regular and then how often did your periods come um normally every 28 days okay and then were they painful no okay and was the flow light moderate or heavy moderate okay and then how many days did the bleeding last for about five and then again looking at the calendar when was the last day that you had intercourse um and again today we're at the ninth nine I would say about March the 23rd and did you use a condom yes okay and then when was the last year that you had a pap smear um August of 2011 12 okay and was were the results normal abnormal or did you find out and from day were normal okay mm-hmm I'm just gonna go back to your pregnancy history so you said you've been pregnant five times yes of those five how many resulted in full-term births greater than 37 weeks three and then what are the birth dates of your children um December 24th 1999 all right and then we have January 2003 all right in May 2005 all right and then were these Badgers deliveries or c-sections vaginal all vaginal yes okay and then of those five pregnancies how many resulted in premature births less than 37 weeks no okay how many resulted in miscarriages less than 20 weeks yeah how many resulted in stillbirths greater than 20 weeks no okay how many resulted in elective abortions - okay and then what were the month and years of those procedures um first one would be August of 1992 and the second one would be May of 2009 and then we're those surgical or did you take the medication and go surgical okay and then how many of those pregnancies resulted in ectopic pregnancies or a pregnancy in the tubes oh no okay and so now we're going to talk about your history of any past infections have you ever been told you had gonorrhea no okay what about chlamydia yes okay and what month and year was that um that would be November of 2008 November in 1993 Majan I three okay and were you treated for that yes okay have you ever been told that you had trick or trichomoniasis no syphilis no genital herpes no genital warts no HPV or the human papilloma virus no okay what about BV or bacterial vaginosis a bacterial infection no okay and then are you allergic to anything any medications latex or food not at all what about shellfish no okay and then are you currently on any medications yes I take um have blood pressure medication Lynn sit up real well okay and then what's the dosage on that uh five milligrams okay and how many times per day wha stayin okay any other medications that goes in low to pain okay and then what's the dosage on that one ten milligrams and then how many times per day which is nice today alright and then have you had any surgeries specifically to your reproductive organs no okay now I'm just going to go through this long list of medical conditions have you ever been told that you had cancer no what about HIV no hypertension or high blood pressure yes okay and then what month and year were you diagnosed with high blood pressure um I was diagnosed I would say April of 2008 and you said you are currently being treated you study with those with the medications or more okay have you ever been have you ever had a heart attack no what about a stroke no okay migraine headaches no okay high cholesterol No blood clots and your legs our lungs nope okay what about diabetes no did you have diabetes when you were pregnant no okay bye Roy problems no liver disease no PID or pelvic inflammatory disease no okay any abnormal vaginal bleeding no okay any uterine fibroids yeah or uterine abnormalities no okay any depression or anxiety yes okay which water both and depression okay and then what was the month and year you were diagnosed with depression um it would have been July of 2009 all right and then are you currently being treated or would you say that that's resolved it's resolved all right and any other medical conditions that I didn't mention no you didn't think of no okay great all right so that is it for the medical history so now I'm going to go through
