[Music] hello everyone in this tutorial we'll be learning all about the template designing step or how to design a tooth supported guide in order to do that you'll need to see this icon in your version of msoft this is the step 7 template button once we click on this button we can see that our model changes to a green color it's best to look at this using the occlusal view there are several steps that we need to go through in this design process the first step is setting our path of insertion this is just like surveying in dental technology where in this case we'll be determining the undercuts if i click here to define the working direction we can now see the different colors on the model green means no undercut yellow means neutral and red means undercut by changing the position of the teeth i can control the amount of undercuts here i select the equator of the teeth that i want the equator is the part of the tooth that extends the furthest out in the buccal and lingual directions i'm trying to see as much tooth as i can because i want a balanced path of insertion if i click again we can see that the red area has gotten a little bit higher because i change my path of insertion slightly the next step is clamps the rule of thumb here is that the more clamps we have the more support we'll have and the tighter the fit will be for the guide what i usually like to do is place all the clamps first and then go back and adapt each one individually here we have a lower toolbar note that in this toolbar we have our tube and our foot distance i like to set this all up beforehand so that i don't need to go back and change the distances in diameters individually for most of the clamps i'll use a 2.85 to three millimeter distance let's take a look at how we change the default parameters go to the application settings and navigate to the second page on this list you can see all of the parameters of the guide including initial clamp diameter and distance setting these initial parameters will save much time later on when you get to this stage of template design so now you can see that all the foot distances are at 2.85 the tube diameters are at 3.6 millimeters teeth we need to set up our feet in a way that the tube does not go into our tooth or gingiva here you can see that i've placed this slightly into an undercut to make things go faster i suggest doing all of the buckle placement of the teeth and then continuing on to do the lingual section you can see here i'm setting it up so that the tube doesn't make contact with the teeth i can move the model around to look at it occlusally and set up my foot on the tooth another recommendation is to always put the feet on a cuspid in pre-molars or molars and not on a developmental groove this will give you more surface area to work with and is also higher up to turn the model as i'm doing now i click on the shift and hold the mouse button while turning i can move the parts of the feet by holding down the left mouse button if i decide that my feet are too wide i can change the diameter here it's important to note that what is active is colored orange while all the other items are blue in other words if i want to make changes to one of the feet i need to activate it and make sure it's orange only one clam can be active at a time let's make some more refinements and make sure our tubes don't go into any teeth we also need to check that our clamps go around and not into the teeth as i've already mentioned the more feet you have the more retention you have i usually set the foot distance at 3.75 millimeters and for the foot diameter i go with a 3.2 the width of the tube is now set at 5 which should be strong enough but if we wanted it stronger we could increase it the next step is to add the lingual wire i'm adding two feet here so it can go around you can add one or two connections to the sleeve depending on if you have the room for it usually in a posterior tooth you can have just one foot connection i'll put two here just to make it stronger i'll just add another one here you don't need to put it in the same spot that you have a clamp placed you can go in between here you can see that this is going to be connected even though we don't have a foot here as long as it's inside then you know it's going to be connected because they overlap i'll just add another connection here make sure not to place these connections on the gingiva this will hurt the patient and will not have any function that will facilitate the procedure for even more retention i can add another foot here i always try to have the feet as far away from the sleeve as possible remember that we're trying to get a contra angle in there so if i feel that this might interfere with the contour angle then i'll leave it blank our next step is to create a buckle wire with the buckle wire we don't want a distance less than three millimeters otherwise it will get stuck in the patient's teeth i'll just set the foot diameter at 3.2 millimeters on the buckle always remember to place two attachments this isn't necessary for the lingual or palatal but it's critical for the buckle here i'm also putting the feet on the teeth i don't need to put them on every tooth because we already have some from the clamps this is our guide from the occlusal view i can be sure that the bar is not touching teeth if it is i can just make the foot distance larger you can see that it's now further away with more retention we get more stability here from the occlusal view we can see that neither the lingual nor the buccal bars are making contact with teeth or gingiva our next step is creating arches arches are connecting pieces that go from one side to the other if i place an arch here i make sure it's elevated to make room for the tongue on this specific arch we can print the name of the doctor or the patient so that we know who this belongs to these can be attached anywhere sometimes i like doing arches like this so that there's more room for the patient's tongue i can also do a tongue guard to keep the patient's tongue away from where the dentist is placing the implant i click on alt and add in some more points also if i have a scan post set up i can mark where i want the flat side of my implant to be by putting in two markers as i've done here i'll then make the diameter two millimeters here i have it marked but i need to make sure that it's two millimeters or less otherwise it will interfere with the ratchet when tightening here you can see that i've made this tube thinner or we can thicken it up again to four millimeters if you want to add in the patient's name where you start writing will determine the direction the name will be written in the next option is for macosa support which we don't need in this case because we've said everything on existing teeth mucosal support is for dentureless cases this case is tooth supported we can do something interesting with this option however which i'll show you now let's first get rid of this clamp here we'll use a mucosal support on this specific tooth some reasons for doing this may be that the patient's mouth doesn't open very wide or perhaps there is no room for a clamp or i would prefer not to have a clamp here after using the mucosal support option on just this tooth i'll bring the thickness down to 1.5 because 2 millimeters is just too thick and now i'll attach these two arches here and here later on when we do our calculation this mucosal support will give us a wide support and strengthen this area substantially here we have our sleeve in this case we only have one sleeve here's the toolbar for the sleeve i can put museo because we can see that the sleeve is close if i put my cursor in a corner and click the alt button i can rotate the sleeve in a direction that suits me i can also subtract from the width of the plastic that's going around the sleeve making it any width i choose i can also add ovality this means strengthening the buccal lingual layers as opposed to the mesial distal areas the sleeve has an upper half and a lower half if i want to make the lower half thicker i can click here now the lower half is the same thickness as the upper half 0.25 is as thin as it gets i can trim the plastic or choose not to after printing during the try-in phase let's click back on the model and flip it over this last function is where you can put the patients or doctor's name clicking ok saves the name we've put in i believe that you can enter up to 12 letters last step is the calculation step clicking here will calculate the guide and turn it into a solid stl file once this is done the case is locked and cannot be changed if you have made a mistake and noticed it in the calculation you can
