Hello, I'm Jennifer Woodward, state registrar for Oregon Vital Records, and I'm here today to share some information about a new service. Starting January 1st, 2018, people who were born in Oregon and who need to change their name or sex on their birth certificate to reflect their gender identity may do so by completing a notarized application rather than having to get a court order. The first step is to complete an application to change the name and/or sex on a record of live births to support gender identity. The form is on our website. We can only accept forms signed and dated on or after January 1st, 2018. Please fill out this form carefully and accurately as it will become an important document in establishing your new birth record. The form must be notarized. The second step is either mailing or bringing into our office your completed application, paying the required fees and ordering a new birth certificate. The cost is $35 to amend the record and $25 for each certified copy of the new birth certificate. You can mail your application, fees, birth order form to Oregon Vital Records, P.O. Box 14050, Portland, Oregon 97293, or you can come to our office at 800 Northeast Oregon Street, suite 225. We are open from 9:00 a.m. to 4:00 p.m., Monday through Friday, excluding state holidays. The birth certificate with the new name and/or sex designation will be ready 7 to 10 working days after the application, the fees and the order for the birth record have been received in our office. Along with the new birth certificate, you will get a copy of the application and a letter from the state registrar approving the administrative changes to the birth certificate under the new law.
