welcome back to the NASM how-to series for online straining my name is Karin and today I'm gonna be showing you guys the forms that I require with my online personal training client I also have some optional forums and I'll be going over that with you guys in the next episode so the first thing is when a client comes on board with me they've reached out to me and they've expressed interest in online personal training after we do our consultation call I send them an email that has all the information about getting started basically everything we just talked about on the phone but within that is their contractual agreement and that is where I state my 24-hour cancellation policy which simply states that if they were to cancel their session within 24 hours without enough notice that they will forfeit their session now that's something that I recommend you do as a trainer regardless of where you work whether it's online in home or at a gem but especially online people at first tend to think of it like no big deal to cancel but when you truly think about it I mean this is your time and this is your job and that's a spot you could have given to another online client not to mention this is your income and so you rely on that I have found that clients are pretty respectful of that if you really hold them accountable and they know they've signed off on it and the other thing that's really good about 24 hour cancellation policies is it keeps the client going and accountable when they would maybe rather cancel their session just out of laziness so they don't feel like doing it so for those reasons you want to go ahead and charge your session through that side there are exceptions of course family illness and emergencies those kind of things I do waive the 24-hour cancellation policy for those types of situations now the other forms that I require I have on my website and I just have a private link that they can go to now for liability and for health history you can find those kind of thing online and send them over to your clients if you want I'm going to show you in just a minute what it looks like on my website now the park you use so the physical activity running this questionnaire and ASM actually provides that for you if you want to go to their website you can also find others online I created my own and then for a liability form and you can also find those online if you don't have a relationship with an attorney but what I did is I actually did a few years in have one write it up for me just because my clientele was growing so much with online training so that's just something you'd want to consider so just streamline it a little bit for me I was just getting so busy actually hands-on with clients and just to keep me from having to send more emails back and forth is when I got this link but just goes straight to my website once they fill it out it can get stored in their online file and I don't have to deal with it so that's something you can look to do in their future so this is what the page looks like once I've sent them the forms you'll see down here the measurements forms and the before photos or what we're going to discuss in our next episode because those are optional so these are the required forms I do not work with a client unless they have filled these out and you shouldn't be either whether that's online or in person so required liability form you can see they can put their name they can read everything and then towards the end they do have to sign and date and submit and then that will cover you in the case that they were to be injured I have never needed that but I hope to keep it that way it's good to have it covered health history form here so again just like any of the other parties you just ask any conditions they might have medical conditions hospitalizations surgeries vitamins medications other things you might need to know about I asked about their health habits personal safety dietary habits how are they exercising all those kind of things so you can kind of customize that and personalize it for however you need it for you and your clients and then the other thing form one is is how you're going to handle their workout log so as I stated before in a previous episode I Google Drive and they are able to receive that in real-time but there are a lot of options out there so just find whatever is going to work for you and your clients regarding their workout forms but as far as legal documentation these are absolutely required if a client is going to work with me for online training so just make sure you're protecting yourself in your business as well as your client
