you about electronic medical records it may be stating the obvious but medical care in the United States has changed in three major ways in the first place large medical centers are buying up and consolidating medical practices in their area number two government incentives have made electronic medical records commonplace and number three hospitals are now hiring doctors called hospitalists to take care of you while you are an impatient and these hospitalists may not and probably do not know you or the other doctors who care for you it's great if your doctors are all within that medical system and all use the same electronic medical records not so great if they don't for example if you see a myeloma specialist at a large medical center or academic center and your doctor closer to home your local oncologist to it actually administers your treatment is not within the same academic center this can pose problems in communication and it throws the responsibility back on to you to make sure that both of your doctors know what's going on even within the arms of a single large medical institution not all electronic medical records are kept in the same way and certainly in different institutions different electronic medical systems are used and they don't speak to each other remember the hospitalist your doctor does not get courtesy privileges and it's not going to be able to visit you in most cases here's a personal example of a bad experience that occurred that could have been prevented my father-in-law Jack was admitted to the hospital for removal of a carcinoid tumor this hospital was not the one where he was normally cared for by his primary care physician or his other specialists when he entered the hospital he brought with him in his hand his medication for high blood pressure he had very successful surgery but the hospitalist who was caring for him post surgically did not know that he had high blood pressure had no communication from the other doctors at the other medical center and did not know that he was supposed to write orders for the nurses to give my father-in-law the high blood pressure medication as a result he had a debilitating stroke that left him unable to walk talk or swallow he recovered beautifully from the surgery but he was permanently disabled by the stroke here's what I learned we have to do as family members as pay on patients to prevent a tragedy like this from happening get copies of all your medical records if you can get them electronically from your health care system and they're available to you easily make copies on your computer desktop save them and put them in a folder on your computer if you can't get them electronically and you can only get them on paper that's fine you request them you have legal right to them if the doctor's office is going to charge you for the copies that's fine pay for it it's worth it then take your digital camera or your phone or a an iPad and take a picture of each of the records and save it on your computer and then put it in a folder on your computer desktop then write a cover letter it should include your name date of birth the names addresses and telephones of all your physicians for all of your medical conditions and your general practitioner and then you should include a list of all of your medical conditions when you were diagonal and what your diagnosis was make a list of all your medications how much you take the days that you take it and how long you have been on each of the drugs that you receive in addition to your prescription medications you must include all vitamins minerals and supplements that you're taking very often these are just fine sometimes they interact with each other or with about other medications that you're taking so you need to be very careful about including all of them whether it's vitamin D or selenium or bite whatever vitamin put it on the list and make sure that it's included with all your other medications right as complete a summary of your treatment history if as you can we'll make this specific to myeloma but you should do this for other illnesses that you're being treated for as well we need to know when you were diagnosed and what your exact diagnosis was if it was smoldering myeloma or active myeloma or M Gus the doctor will need to know that in addition we need to know diagnostic test results that were done including any genetic testing that will influence treatment choices we need to know what crab criteria were present when you were diagnosed that is the level of your serum calcium your kidney function your whether you were anemic or not meaning that you had low red blood cell count and what the state of your bones was when you were diagnosed and in addition we're going to need a very complete treatment history which tells which drugs you had in which order in which combination and beginning and ending dates and this is very important it's a lot to remember and to put down but it's it will really really help you add the cover letter and the treatment history to that folder you created on your computer desktop now put all of that onto a memory stick you put it in your USB Drive and you just drag that medical folder onto a memory stick now do it again on a separate memory stick you keep one with you at all times when you visit the doctor or if you have to be admitted to the hospital that will be on you and it will provide all the background that the doctors at the hospital need work and other doctor needs the second memory stick is to keep it home or to give to a loved one who is helping to be your caregiver you'll have a backup just in case you don't have yours with you at the time you need it each time you see a doctor or have to go to the hospital bring that memory stick with you and let the doctor know that you have a complete record of all of your medical information not only will your doctors appreciate the fact that you have prepared your information it will also be a huge benefit to you and if you don't have the ability to take digital photos and you don't have a computer and you have to work all on paper that's fine bring a folder with you with your papers in it just make sure you have copies of those medical records make the cover letter and make the treatment history and make the list of medications keep it with you in a folder and bring it with you if you have to be admitted to the hospital or if you see another physician it can still work in the old fashioned way
