breast radiotherapy is the standard of care after surgery for most women with early breast cancer that's been established through many studies and reviewed by the early breast child collaboration group we know if you give radiotherapy after breast conserving therapy then overall you halve your overall risk of recurrence but you know two-thirds reduction in the risk of a local recurrence for early breast cancer depending on what your risk is so for years the standard of treatment was to give a five-week course of radiotherapy so giving a dose of 50 gray and 25 what's called fractions or doses previous studies in canada and the start trial here in the uk showed that if you gave a slightly bigger dose each day but a slightly lower total dose what's called hypo fractionated radiotherapy then the sort of three-week course was just as effective at reducing the risk of recurrency of breast cancer and actually resulted in slightly less long-term toxicity for women such as breast shrinkage hardening and um so that was established as the standard many years ago now this study was trying to push further we did another study called the fast study trying to work out what the best sort of compromise was between sort of toxicity and long-term outcomes as far as recurrence goes so and that then led to the false forward study which is this study so in this study there was several thousand patients so we wanted to uh find us a regime that would be at least effect as effective as the three week ray regime in terms of reducing the risk of local recurrence and also long-term toxicities so we didn't feel the study needed to be better the results better we thought well if you could concentrate and give the treatment in a week with no inferior outcomes that would be a a very good outcome for women and health care systems so trying to find the best dose it's always a compromise between toxicity and outcomes so there were three arms in this very large study we tried to have a sort of pragmatic design so that it was relevant to the population of women globally with breast cancer so anyone who'd had either a wide excision or a mastectomy with some nodal surgery who'd had clear margins and they were generally well they were randomized to three arms either the what the standard of care which is a 40 grain 15 doses over three weeks or two experimental arms one giving 27 gray in a week and one giving 26 grams a week and the primary outcome was was local regional relapse with so the usual secondary outcomes of in the regional will have some metastatic disease toxicity as well and as part of the study we did some sub studies so we did acute toxicity and two groups of patients to see whether there was some less toxic more toxic or no more toxic that...
