2. The name of the applicant;
3. The employee's last position;
4. A description of the duties of the employment, in the job description on the company website, if available.
5. Proof of a valid work permit for the United States or the EU (for example: a valid driver's license for a permanent resident);
6. The employer's address with the postal code (for example: "PO Box 10-14, New York, NY 10011").
7. A copy of the employer's signed "Statement of Relationship" (for example: "This is for my sister-in-law who is in the United States.");
