hi we'll be walking you through how to fill out the association remittance form if you've been the treasurer or bookkeeper at your church for any length of time you know about the remittance form that should be included with your church's contributions if you're new to the job this information should have passed along to you in any case we're going to take a look at how to fill out this form you should have a copy of the form it looks like the one displayed here on the screen if not you should be able to get one from your church office you can also get one online at eoa wraucc.org under resources depending on the association your church is in your form will have either the eastern ohio or the western reserve association logo in the top center of the form if you're not sure which association your church is in please check with your church office for our purposes here i'll be using the form that has the joint logo this form is a fillable pdf you can print it out fill it out by hand and mail it in with your check or you can fill it out on your computer print it and mail it with your check whichever works better for you the first field is the name of the church just type or write in the name if you're filling the form out on your computer you are able to tab from one field to the next the next field is the church number this number is four digits long if you're not sure about your church number please check with your church office or call the association some of you may have a six-digit number just to clarify the 5-2 at the beginning of that number designates the ohio conference the next four digits are your actual church number the next field is the church's city this is an important field it helps us tell which of the 43 first congregationals or 62 saint john's we're talking about the next section is for contributions to ocwm our church's wider mission and the four special offerings that are held throughout the year you can also designate a contribution directly to your association just tab to the appropriate field and enter the amount of the contribution the third section is for other contributions we encourage you to make these contributions directly to the organization it can take six to seven weeks for the donation to come from the church to the association office and for the office to process it and get it to the organization it's nice to get that money where it's needed as quickly as possible and it will if you send it directly there is a list of the most common benevolences and their addresses on the website under resources the left side of the section is for gifts you want us to forward on your behalf the four most common benevolences are listed and there are spaces for you to type in others just enter the amount and if needed the name of the organization the right half of the section is for gifts you sent directly to an organization you're welcome to share that information with us if you like the fourth section is for your check number and the total amount you are sending to the association and don't check my math here i'm just using this for demonstration purposes there all done not too painful didn't take too long we appreciate you taking the time to fill out and include this form with your church's contribution it helps us better document your giving and leaves less likelihood of error if you have any questions please call 330-940-2220 extension 100 or email support.center at eoawraucc.org thanks for watching this video and we hope it was helpful you
