hello phoenix parents this is mr duncan and i'm making this video to demonstrate to you how to sign a permission slip for momentum education now we're going to start with the document itself so you're going to receive this document via email you're going to need to download it so i'm going to download the document now once i see it down in my bar my download file i'm going to click on file explorer down the bottom smooth as you can see so i clicked on file explorer now i see my most recent file here is permission slip english i'm going to double click on this doing so will load in internet explorer or internet edge i guess the permission slip now we wanted to do this this is perfectly good and because we've loaded it in here we can now use the pin option so here's the permission slip i read through it tells me what's going on i'm giving you consent for momentum to match up your student with a college mentor which is what we want we continue reading and at the bottom second page i'm going to fill in this information i'm going to click in the date and i'm going to put in the current date okay maybe today's the 16th okay i'm going to type in uh the name so if i were the parent one okay cardio or whatever my my project my name is uh relationship i can put father here or if i am maybe the grandparent all right grandfather whichever the case might be you put what your particular relationship is um your preferred language i mean you can put either english or spanish whichever is your preferred language and it doesn't matter if you're doing this on the spanish form or on the english forum you can fill out either form and then specify your preferred language and then your signature now for the signature this is where we're going to use this is why we've opened this document in internet explorer or edge is because in the top right corner there is an option for me to uh sorry yeah yeah that's correct to add notes this feature also allows us to now draw on the document and then we can save this so this is where we're going to sign okay so i've come up here and i'm going to click on this now i want to have lots of space when i write because i write a little bit large so i recommend that you zoom in first on the signature and if you don't you can also if you're using your child's device use your fingers and pinch the screen out outwards like that and if i do so you'll notice that makes my screen get larger if i pinch inwards that'll make it shrink so i can resize as necessary all right so now i'm going to go up i'm going to click on add notes and this gives me then some writing functionality we want this touch writing he knows it's automatic we want that on that allows us to write without having to use like a mouse or hold down the click we're going to make sure this ballpoint pen is selected uh it's already got black selected now the font is really fine that's perfectly good if you need to go up to maybe a two that's okay as well probably don't want to go to a three it's a bit thick so one's fine uh two is also great those those would be your choices all right i'm now going to use my finger and i'm going to put my finger on the screen and write out the name i'm going to write my signature so here we go okay and now i've made my signature and if i'm happy with this i'm going to then save this document okay and i can do that by going up to the top right there's a little uh save icon that's that hard floppy disk that we remember and we're done if you don't like your signature you can undo this you can go up here and also erase it you can say hey i don't like this so you should have the erase option i believe let's see if that'll let me erase should be an erase feature right there sorry guys uh let's try that one more time yeah that should i don't know what's now as odd usually allows us to erase so maybe i've already i think i've already saved it that's probably why so i then have saved my documents because i'm ready to go so i don't need to do anything else from here this is this document's ready to go it's been saved so i'm going to close out of it now just to make sure that that got saved i'm going to reopen it just so you can see that it is in fact now signed so we scroll down there's all of our information and there is my oh it did erase okay so apparently this didn't show up on our screen but it did erase that first name that i asked it to so um just be aware of that it's good to check your document um if you want to you can also rename it make sure you know that way it's easy to find for when you upload it so i probably should put here signed and comma and then i will put the name my name right which of course i'm not juan cody yo but that's okay it's good for demonstration purposes so i put signed parent name and then i would just hit save this is the english version that's why it says english on the end if you're signing the spanish version it'll say espanol or spanish on the end okay hit save all right so we've got that part done and we're happy with it and again if we're not we can always go back and we can do some quick edits all right and i can go add that name back in and if i'm okay with it okay save it's ready to go i'm gonna close out from here you should have also received a form to fill out this submission form so i'm going to load the version that you'll see that'll be here so this is the form permission slip uh submission this is where you're now going to upload your um signed document so this is just the banner just kind of shows the different versions in english and spanish um [Music] it should show the same thing right below yep so uh make sure you start filling this information in so put your email address in this is mine of course you can put your personal email address in here it's fine next down below choose the school year this is 2021 we're going to be doing this every year so that's why there's more options but the name of your student okay so uh perhaps we will say okay so i've got my student name in and then put my students email address my students alden email address so all the email addresses um for students that might be something like cc and then their six digit number let's see that's one too many digits i think we'll take that off at now if you don't type this in correctly it's like oftentimes people forget to put the dash in and i try to move on it says no because it should have a dash in there so i go back and i go okay yeah and if i actually put a gmail account in here it's also going to come up with an error because it's going to say no i want the one that's got an at alden isd isd.org in it so be aware uh this is just to help keep track of who signed the document of course so here i would put the name of the parent who signed the document and is uploading it i'm clicking clicking english or smash this allows those who are processing your form um you don't have somebody who could read spanish appropriately or english you know so it just makes it easier for them to know okay that's which form he's been uploaded so choose the form you used doesn't matter which one you use and then you're going to click add file all right so we can drag it in by again opening up our file explorer just move a little bit over and i can click on my signed permission slip and drag it over okay and that's it i hit upload and now we hit submit and we're done that's all you need to do so the process should take you maybe five minutes right much less time than it's taken me to explain how to do it but the big point is of course fill out the form sign it i'm using the touch screen you can also fill it out on your phone that's totally fine once you've signed it don't email it back to mr duncan i i love my parents i love our envelope's parents but guys there's 255 of you and my emails are already are way backed up so that's why we're using a google form to capture it it helps us keep track of who still needs to submit and we can keep up with those parents and those students and allows it to all be organized and easy for you guys to to put in one place all right so then you just hit submit and you're done okay so i hit submit and it's complete all right and that's it this of course takes it back to it says your response has been recorded you shouldn't either submit another response you only need to submit one for this year okay and then if you do need to submit another response you can do so um you know next year and if there's something wrong with the response you submitted that's what this another response means you can go back in maybe you're like oh i forgot i upload the wrong one what do i do just submit another one that's it's easier if you do want to edit the response you sent you should receive an email receipt saying hey we got your your response your form here's a link so you can go back and edit if you need to so that will be in your email all right that's all thank you so much
