Like a PDF form? You can’t edit a PDF form without an app to do so. Even then it is best to have the original form such as a Word DOCX and recreate the PDF. If the PDF has security configured it will block your attempts to alter it. You should be able to fill it out if it is a PDF fillable form then print it, etc. If it’s not a PDF then it’s an image and cannot be edited. Could be they want you to print and fax it. Not very modern but it still happens a lot even today.

Each question is asked on a separate screen so patients won’t miss anything. They can only progress to the next screen once they’ve filled in the current screen. Once the form has been filled out you can produce a PDF and store that in your patient management system. It’s super-configurable: ask any question, in any order – virtually any type of form can be made . You can get their photo and their signature. You can only ask if they could be pregnant if they’re female, rather than always like paper forms.
